A careful history can change which questions we ask.
Is This For You?
You Might Be Our Ideal Patient If...
If symptoms persist or the diagnosis feels incomplete, a closer review may help clarify the next step.
Tried Multiple Medications
You've tried medication without enough improvement and want a closer look at the diagnosis, treatment history, and medical factors.
Exhausted Despite Sleep
You wake up tired despite enough time in bed and want sleep, medication effects, and medical history reviewed together.
Brain Fog & Focus Issues
Concentration problems, memory issues, mental sluggishness—and you're wondering if it's ADHD or something else entirely.
Anxiety That Won't Quit
We review racing heart, persistent worry, and physical tension alongside your sleep, medication history, and medical history.

Founder and Clinical Team
Horizon Peak Health was founded by Canybec Sulayman, PMHNP-BC, MBA. The practice now includes Sharon Hernandez, PMHNP-BC, a California psychiatric-mental health nurse practitioner whose clinical interests include mood and anxiety disorders, trauma and recovery, adult ADHD, LGBTQIA+ mental health, and perimenopause.

Canybec Sulayman, PMHNP-BC
My 19 years across 7+ ICU specialties shape how I review medical clues alongside psychiatric symptoms. When a treatment isn't helping enough, I want to understand the pattern before changing the plan. What else could explain what you're feeling?
Meet Canybec
Sharon Hernandez, PMHNP-BC
Sharon provides trauma-informed psychiatric care for California adults. She pays particular attention to the overlap among mood, anxiety, trauma, substance use and recovery, ADHD, identity, sleep, concentration, and the mental health changes that can accompany perimenopause.
