Gilbert, Arizona
Adult ADHD Evaluation in Gilbert
A 75-90 minute evaluation with Canybec Sulayman, PMHNP-BC, that checks the childhood pattern, the current pattern, and the medical mimics (thyroid, iron, sleep, medications) before anyone writes a prescription.
Who this fits
Focus problems started early, worsened under pressure, or became tangled with sleep, burnout, anxiety, mood, hormones, or medication effects.
Visit format
Gilbert in-person care and Arizona telehealth are available when clinically appropriate.
What to bring
Bring childhood pattern, school or work history, sleep pattern, current medications, prior trials, labs, and daily-function examples.
Our Approach for Gilbert Residents
Why Focus Problems Need Medical Investigation
Many Gilbert residents struggle with focus, concentration, and productivity and assume it must be ADHD. A symptom checklist alone can miss medical contributors such as thyroid dysfunction, iron deficiency, sleep disorders, medication effects, or nutritional deficiencies.
Before diagnosing ADHD, we investigate whether thyroid dysfunction, iron deficiency, sleep disorders, or other medical factors are contributing to the symptoms. If a medical driver is present, the treatment plan may change before ADHD medication is considered or adjusted.
Your evaluation includes labs and a medical review when the picture calls for them, not a checklist and a prescription. Canybec spent 19 years in critical care nursing; the habit of checking the body first came with him.

These conditions cause focus problems identical to ADHD—and won't respond to ADHD medication
Thyroid Dysfunction
Even mild hypothyroidism causes cognitive slowing, brain fog, and difficulty concentrating
Iron Deficiency
Low ferritin impairs dopamine synthesis—the same neurotransmitter targeted by ADHD medications
Sleep Disorders
Sleep apnea and insomnia create symptoms identical to ADHD
Vitamin Deficiencies
B12, folate, and vitamin D deficiencies affect cognitive function and focus
Our Process
How We Approach ADHD for Gilbert Patients
Screening call
A free 15-minute call (Monday-Friday, 8 AM-5 PM Arizona time) to confirm fit. If you're using insurance, benefits are checked before the visit.
Evaluation
75-90 minutes with Canybec Sulayman, PMHNP-BC: childhood and school history, how focus behaves at work and at home, sleep, mood, medication and substance history, and rating scales as one input rather than the whole answer.
Medical check
Labs for thyroid, ferritin, B12, and vitamin D when the picture calls for them, plus a sleep screen when snoring or unrefreshing sleep is in the story.
Plan
ADHD treatment when ADHD is the diagnosis; treating the mimic first when it isn't. Medication decisions, including stimulants, follow the evaluation and the medical check. 30-minute follow-ups track response and side effects.

Common Questions
ADHD Evaluation FAQs for Gilbert Patients
What makes an ADHD evaluation at Horizon Peak Health different from other Gilbert providers?
A symptom checklist is one input, not the answer. The evaluation is 75-90 minutes with Canybec Sulayman, PMHNP-BC. When clinically indicated, it includes labs so that thyroid function, iron stores, B12 and vitamin D, and sleep can be reviewed alongside the ADHD question rather than assumed. Testing is individualized, and anything found is addressed as part of the plan alongside the ADHD question, not instead of it.
How long does an ADHD evaluation take?
75-90 minutes. It covers childhood and school history, how focus behaves at work and at home, sleep, mood, medications and substances, rating scales, and the medical history. Labs are ordered when the picture calls for them. Follow-ups are 30 minutes.
Do you accept insurance for ADHD treatment in Gilbert?
If you're using insurance, you pay your plan's copay or coinsurance. Call (480) 535-6455 to verify your plan before the first visit. Benefits verification is not a payment guarantee. Self-pay is $400 for the initial evaluation and $150 for follow-ups.
Can thyroid problems cause ADHD-like symptoms?
Yes, thyroid dysfunction can cause cognitive slowing, difficulty concentrating, fatigue, and brain fog. This is why thyroid testing may be part of the evaluation when clinically indicated.
How do I get to your office from Gilbert?
The Gilbert office is at 2451 E Baseline Rd Ste 310, Gilbert, AZ 85234. It serves Gilbert and the surrounding East Valley.
Do you offer telehealth ADHD appointments for AZ residents?
Yes. In-person visits happen at the Gilbert office, and telehealth is available for Arizona residents when clinically appropriate. Initial ADHD evaluations are usually in person; follow-ups are often a good fit for telehealth.
Will I leave the first visit with a stimulant prescription?
Not automatically. Medication decisions, including stimulants, follow the evaluation and the medical check. When ADHD is the diagnosis, treatment is discussed and started when appropriate; when a medical contributor such as low ferritin, thyroid disease, or a sleep disorder is part of the picture, it is addressed as part of the plan. Never start, stop, or change ADHD medication without your prescriber.
How much does an adult ADHD evaluation cost in Gilbert?
Self-pay is $400 for the 75-90 minute initial evaluation and $150 for 30-minute follow-ups. If you're using insurance, you pay your plan's copay or coinsurance; call (480) 535-6455 to verify your plan.

"Good help is hard to find. And I'm incredibly grateful that I found Canybec because he's one of the best. He instills in you a curiosity about treatment so that you yourself can get a really good sense of what meds and dose are right for you. It's as engaged as I've seen any clinician. He has reached out to me outside of sessions, giving me ideas on other things that can help. I appreciate the holistic approach that he takes. Lastly, it's incredibly clear that he actually cares. If life circumstances get hard and things aren't working for you, Canybec will work with you. He tries to find creative ways to make things as cost-effective for you as possible. Can't thank him enough for all his help."

"Just wanted to thank Canybec for help in managing my mental health needs. After an unpleasant experience with my PCP who initially managed my mental health needs, I reached out to Canybec who was very knowledgeable, patient, and empathetic. I informed him of the side effects I was having on a certain class of medications. He explored and offered different medication and treatment options - even showing latest evidence-based practice and research for each option. Thankfully, the option I've chosen has been working great for me without the major side effects and allowed me to be functional again."
