SMS Terms
Texting is optional. You can submit a website request or book a discovery call without agreeing to texts.
What you are agreeing to
When you check the SMS consent box, you give Horizon Peak Health permission to send one text acknowledgment about that request. This permission does not enroll you in marketing or an automated follow-up campaign.
For a clinician referral, the referring person must confirm that the patient agreed to a text at the patient's number. Permission to share the referral is a separate choice.
Rates and opting out
Msg and data rates may apply. Reply STOP to opt out. You can still contact the team by phone or submit a request without SMS consent.
Keep medical details out of ordinary texts
Ordinary SMS is not secure messaging. Messages may be visible to anyone with access to your phone. Do not send medical records, insurance card images or other sensitive details by text. Ask the team for a secure way to share them.
A text acknowledgment does not confirm an appointment or a clinical decision. These messages are not an emergency service.
