Most founders drink more than they tell the first clinician.
Not because they are being dishonest. Because the question is asked once, in a medical context, and the frame is addiction rather than clinical effect. Three or four drinks most nights does not feel like addiction. It feels like a functional professional who knows how to decompress.

The Fast Answer
- Alcohol reduces anxiety acutely for two to four hours. It then produces rebound anxiety — often called "hangxiety" — that is mediated by GABA/glutamate rebound and elevated cortisol.
- Regular drinking fragments sleep. It helps with sleep onset but reduces REM sleep and causes middle-of-the-night wakings as blood alcohol falls.
- Chronic alcohol use activates the HPA axis — the stress hormone system — and produces an elevated baseline anxiety that looks indistinguishable from generalized anxiety disorder.
- Alcohol impairs antidepressant and anti-anxiety medication response. A patient who is drinking regularly may appear treatment-resistant when the core problem is that the alcohol is maintaining the symptoms the medication is trying to treat.
- Alcohol is a CNS depressant. At the quantities founders commonly drink, it can produce depressive symptoms — flat mood, anhedonia, low motivation, cognitive dulling — that look like major depression.
This is not a morality question. It is a pharmacological one. The alcohol is doing something specific to the neurobiology, and that effect needs to be named before the psychiatric picture can be accurately read.

Why Founders Drink
The pattern is recognizable and not surprising.
Alcohol is a reliable state change. After a day of high cognitive demand, social performance pressure, and ambient uncertainty, a drink produces rapid GABA-A receptor activation — chemical decompression. It works. It has worked for thousands of years.
In founder culture specifically, alcohol is socially normalized. Investor dinners, team events, conference circuits, and the general ritual of "drinks after" are part of how the professional environment operates. Declining often requires a visible explanation. Drinking is the default.
Founders also tend to have difficulty with downregulation. The pattern of relentless urgency, context-switching, and background problem-solving does not stop at 6 PM. Alcohol is often the most reliable off-switch available.
The problem is the pharmacokinetics.
The Anxiety Loop
Alcohol reduces anxiety acutely. That is real and not controversial. GABA-A receptor activation produces anxiolysis within 20 to 30 minutes at typical drinking quantities.
What follows: as blood alcohol levels fall over the subsequent hours, GABA-A activity normalizes and glutamate — which alcohol was suppressing — rebounds. Glutamate is excitatory. The rebound produces increased heart rate, heightened arousal, and subjective anxiety that typically peaks in the morning after a night of drinking.
The patient interprets the morning anxiety as baseline. It is not. It is pharmacological rebound from the night before.
If the patient then drinks again the following evening, they experience relief from the elevated morning anxiety. This creates a self-reinforcing loop that is genuinely difficult to identify from inside it. The drinking reliably helps; the morning is reliably difficult. The connection between those two facts — that the drinking is producing the morning — requires a clinical conversation to name.
Chronic alcohol use at regular quantities also activates the hypothalamic-pituitary-adrenal axis and keeps cortisol elevated. This sustained cortisol activation can produce generalized anxiety, hypervigilance, irritability, sleep disruption, and low-grade depressive symptoms that are pharmacologically maintained as long as drinking continues.
What Alcohol Does To Sleep
Alcohol reliably reduces sleep-onset latency. Founders who drink to sleep are correct that it helps them fall asleep faster.
What happens next: as blood alcohol falls during the first half of the night, REM sleep is suppressed, and the second half of the night is fragmented. Patients wake earlier than they want to, feel unrested, and attribute this to stress or insomnia rather than to the drinking.
The full picture: longer total sleep time with alcohol than without, but the sleep architecture is disrupted enough that the restorative value is significantly reduced. The patient is spending seven or eight hours in bed and waking with cognitive impairment, emotional reactivity, and energy that looks like sleep deprivation — because it functionally is.
This is the same clinical presentation as sleep apnea: adequate hours in bed with non-restorative sleep producing daytime cognitive and mood impairment. And alcohol can worsen underlying sleep apnea by relaxing the upper airway musculature, making sleep-disordered breathing more likely on nights when drinking occurs.
Alcohol And Antidepressants
A patient who is on an SSRI, SNRI, or any other antidepressant for anxiety or depression and is drinking three to four drinks most nights is running a competing pharmacological intervention alongside their treatment.
Alcohol is a CNS depressant. SSRIs work by modulating serotonin. The CNS-depressant load of regular alcohol use directly works against the mood-stabilizing and anxiolytic effects of the medication. This can look like treatment non-response or partial response.
There are also pharmacokinetic interactions. Alcohol affects liver enzyme systems — CYP450 enzymes — that metabolize many psychiatric medications. Regular drinking can change medication blood levels in ways that are not captured by prescribed doses alone.
I do not automatically assume a patient with partial antidepressant response is on the wrong medication until I understand the drinking pattern. If the patient is drinking at quantities that are pharmacologically active, the medication cannot be fairly evaluated.

What I Ask About Alcohol
I ask about alcohol in every intake — not as a screen for addiction, but as a pharmacological history. These are the questions that matter clinically.
How many drinks per week? On how many nights? What type of drink and what quantity? What time in the evening? Any history of needing to drink daily to feel normal, or significant discomfort when not drinking for two or three days?
That last question is important because significant alcohol dependence carries withdrawal risk that changes the clinical approach. Benzodiazepine withdrawal is the better-known withdrawal syndrome, but alcohol withdrawal — in patients who have been drinking daily at high quantities — can be medically serious. I am not suggesting that most founders are in this category. But I am asking because the answer matters for how any reduction is approached.
For the more common pattern — three or four drinks most nights without physical dependence — the clinical question is different. It is whether the patient has ever tried a 30-day period without alcohol and what they observed about their anxiety, sleep, and mood. Most have not.
The 30-Day Test
I tell patients who are drinking regularly and struggling with anxiety, sleep, or mood: you cannot know what your baseline is while this pharmacological variable is active. An antidepressant trial in a patient who is drinking nightly is a confounded trial. A sleep evaluation that does not account for nightly drinking is an incomplete evaluation.
A meaningful trial of abstinence — long enough for the GABA-glutamate system to recalibrate, the cortisol axis to normalize, and the sleep architecture to recover — takes at least 30 days. The first week is often the hardest: rebound anxiety, disrupted sleep, irritability. The second week typically improves. By week three and four, many patients see significant improvement in baseline anxiety, sleep quality, and morning cognitive function that they had attributed to their psychiatric diagnosis.
This is diagnostic as well as therapeutic. If the symptom picture substantially improves with abstinence, the alcohol was a major driver — and that changes the treatment plan completely.
